Three moments. The client shares the record before you meet, you sign your work into it while you work, and they walk out holding it. We are the rail in the middle and we never get a vote.
They send it once, at intake, and it is theirs already, so there is no form to chase and nothing to re-key. You read it before the call. Only what they consented to share is in there, scoped at the door and revocable the whole time.
Your notes stay private by default. Signing one into the client's file is a deliberate act: dated the day you write it, appended to a record that never edits the past, visible to them, revocable by them.
Nothing is exported and nothing is deleted, because it was never yours to hold. The client leaves with every read and every signed entry, and the next practitioner they sit down with can start caught up too.
A client's first read mints their file. From then on the work compounds instead of resetting: the brief reads from the file, the session writes back, and the file belongs to them.
The client's card is already composed from what they consented to share: the last entry in the record, what changed since you met, the plan you agreed, your own last signed note. Thirty seconds and you are current. The brief stays that way because the work itself writes it.
Signed entries live in the client's own record rather than locked inside practice software you may not be using next year. When the engagement ends, your work travels with the client, with your name on it, into whatever room they walk into next.
Every entry is checkpointed the instant it is signed and the ledger only appends. Nobody, including us, can insert a note dated last month or quietly change what a session said. That is the whole reason a record handed to you by a stranger is worth reading.
The file belongs to the client, not to you and not to us. They can pull access the moment they decide to, without asking anyone, and the revocation is published where you or anyone else can check it.
Every practice tool on the market stores the client's history in the practitioner's account. That is the one thing changed here, and everything else follows from it.

A new file starts thin, and we will not pretend otherwise. Continuity is elapsed time, so a file opened today is worth less than the same file in March. Entries are added, never edited and never reordered. There is no write path into the past, for us or for anyone else.
The ledger publishes signed checkpoints. Anyone can prove an entry existed at a point in time without asking our permission.
A claim we cannot evidence comes back null. An absent record is not an accusation, and we will not let it be read as one.
Every revoked grant is published to a list anyone can read. We publish the count of revocations too, which no one else in this category does.
A grant carries only what the owner agreed to, expires on its own after fifteen minutes, and never carries what they wrote.
